Multidisciplinary Pathway Worker

We are seeking an experienced and motivated Registered Mental Health Nurse, Occupational Therapist, Social Worker or suitably qualified CYP-IAPT practitioner to join the multidisciplinary team within Canterbury Children and Young People's Mental Health Service (CYPMHS). You should have relevant graduate professional training and current registration with the appropriate professional body, such as the NMC, HCPC or Social Work England, where applicable. We also welcome applicants with recognised CYP-IAPT training, including Cognitive Behavioural Therapy or Systemic Family Practice/Therapy. The post holder will undertake assessments, formulation, risk assessment, care planning and review for children and young people up to age 18 presenting with emotional, behavioural, neurodevelopmental and mental health difficulties. You will manage an allocated caseload and deliver evidence-based, child-centred and family-focused interventions within agreed CYPMHS pathways. They will work collaboratively with children, young people, families and professionals across education, social care, primary care and voluntary-sector services. The role requires autonomous practice, supported by regular clinical supervision and the multidisciplinary team. The post holder will also contribute to safeguarding, routine outcome monitoring, clinical governance, quality improvement and service development., You will work within the Canterbury CYPMHS multidisciplinary team, providing assessment and evidence-based interventions for children and young people up to age 18 with emotional, behavioural, neurodevelopmental and mental health difficulties.,

• Undertaking comprehensive assessments of mental state, risk, safeguarding and psychosocial needs.
• Developing formulations, care plans and risk-management plans with children, young people and families.
• Managing an allocated caseload and delivering interventions in line with NICE guidance, CYP-IAPT principles and CYPMHS pathways.
• Providing interventions appropriate to training and competence, including CBT, Systemic Family Practice/Therapy, psychoeducation and parent work.
• Using routine outcome measures to monitor progress and review care.
• Working collaboratively with schools, GPs, social care, paediatric and voluntary-sector services.
• Contributing to MDT meetings, clinical reviews, safeguarding discussions and transition planning.
• Maintaining accurate and timely electronic clinical records.
• Supporting audit, clinical governance, quality improvement and service development.
• Supporting junior colleagues, trainees or students where appropriate and engaging in regular supervision and professional development.
