Full Job Description
The delivery of Community Multi-Disciplinary Team meetings (MDTs), contribute to the Integrated Neighbourhood Teams (INT) model and is one of the functions required to achieve Neighbourhood health in Sunderland. Multi-Disciplinary Team membership includes both health, social care and voluntary care sector providers from across the city, working together to identify patients most in need of a multi professional approach to interventions that help them stay well at home for longer.
The MDT Coordinator provides non-clinical administration and organisation support to facilitate this effective MDTs, working as part of a team to support 37 practices across the six PCNs of Sunderland. The MDT Coordinator provides supports to facilitate effective multi-disciplinary working across the system and to improve patient care.
