As a Navigator in our Mental Health and Social Inclusion Hub, you will play a crucial role in assessing mental health referrals primarily through telephone consultations. Using your exceptional judgment and empathy, you will determine the needs, risks, and urgency of response for each individual. • You will be part of a multi-disciplinary team, primarily receiving calls, processing enquiries and facilitating the triage of people referred into the MHSI Hub team.
• Supervise Assistant MHSI Hub Navigators and support MH Practitioners on the MHSI hub to ensure a high quality service is provided.
• Assist individuals with mental health needs to navigate and access specialist health, social care and community services that will support them in their individual recovery journey
• A commitment to delivering a flexible service which operates 24 hours a day, 365 days a year
