Full Job Description
An exciting opportunity has arisen for a Band 4 Healthcare Facilitator to join the newly developed Mental Health Crisis and Assessment Service (MHCAS) at Lakeside Mental Health Unit.
This role is an exciting and challenging opportunity for the right candidate. You will be required to demonstrate excellent communication skills while providing support and information to a wide range of patients that are in the acute phase of a mental health crisis. The post holder will provide support when someone, both known to services and unknown attends.You will need to have a positive attitude towards Mental Health Care, a desire to want to contribute to the recovery of our patients despite the challenges that may bring and an understanding of some of the needs that may be required for you to provide for on a day to day basis.

Working in these services places you on the frontline of mental health care and will equip you with the skills you need to respond to a variety of clinical needs at short notice and provide you with potential future career development.
Main duties of the job
To meet and welcome new clients that attend the service, providing them with local orientation, getting to know them and their families/carers.
To provide basic nursing care to clients including physical assessments, monitoring food and fluid intake and risk assessments.
To gather collateral information on clients from GP, community teams, liaise with other trusts (SPA/CATT/CMHT) and family/friends, if possible.
To contact medics to support with physical health concerns, assessments and treatments.
To deliver evidence based brief interventions centred on patient's needs, such as personal safety plans, emotional regulation, psychoeducation around drug and alcohol use, sleep hygiene, amongst others.
To take referrals from other teams, ensuring that MHCAS criteria is suitable for the individuals' needs
Be able to communicate appropriately with other teams and within the MDT with comprehensive handovers, assessments and impressions.
Ensure all necessary polices and protocols are followed.
Be able to pro-actively engage with clients in a positive and meaningful manner, with the aim of providing high quality care and support throughout their stay.
