Full Job Description

This is a new initiative to improve placement stability in community settings for people in East Sussex who have been discharged from inpatient mental health services. This is a 12 month role working in the Transitions and Pathways Team but specifically in the East Sussex area. The postholder will work with wider systems across inpatient and community teams and local authorities with the aim of ensuring that services work well together so that patients are less likely to be readmitted to inpatient services.,

  • Work with the East Sussex divisional operational clinical teams to identify patients requiring additional support to maintain their wellbeing in community placement.
  • Develop relationships with a small group of community providers and undertake an analysis of their needs.
  • Provider support, advice and guidance to community providers in order to improve confidence in supporting people with significant mental health needs.
  • Develop relationships with East Sussex Community Mental Health Teams including but not confined to neighbourhood mental health teams, crisis teams and assist in their developing relationships with community providers.
  • Work with the wider Transitions and Pathways Team to make a set of recommendations as to how improvements can be made in the East Sussex area to improve placement stability in the community.

    Registered Clinical Professional Nurse/Social Worker or Allied Health Professional with current registration appropriate to the job role
  • Degree/diploma level supplemented by specialist qualification, training, experience, courses to masters level equivalent relevant to the care group
  • Evidence of post registration training in relevant service area
  • Completion of Mentorship Course/ENB equivalent, Extensive post qualifying experience in relevant service area
  • Experience of supervising staff and students
  • Able to manage a caseload of patients with complex mental health needs
  • Comprehensive knowledge of interventions specific to service area
  • Awareness of Trust policies and service specification, Successful candidates are normally appointed to the bottom of the pay scale unless previous NHS experience in a similar role can be evidenced., You must have appropriate UK professional registration.
