Full Job Description
We are seeking a motivated and compassionate Mental Health and Wellbeing Practitioner (MHWP) to join our Older Adults Community Mental Health Team. This is an exciting opportunity to support older adults experiencing mental health difficulties, including those living with cognitive impairment and/or frailty, within a multidisciplinary community setting.
The post holder will work collaboratively with service users, families, carers and colleagues to coordinate person-centred care, promote recovery, and support positive wellbeing outcomes. Under the supervision of experienced mental health professionals and a Clinical Psychologist, the successful candidate will deliver evidence-based, psychologically informed interventions aligned to cognitive behavioural principles, including behavioural activation, problem-solving, emotional wellbeing support, sleep interventions and relapse prevention planning.
This role offers the opportunity to develop specialist skills in older adult mental health while contributing to a compassionate, strengths-based service that values dignity, inclusion, collaborative care planning and evidence-based practice., The Mental Health and Wellbeing Practitioner will work within the Older Adults Community Mental Health Team, supporting older adults experiencing mental health difficulties, including those living with cognitive impairment and/or frailty. The post holder will work collaboratively with service users, families, carers and the multidisciplinary team to develop and deliver person-centred, strengths-based care plans that promote recovery, wellbeing and independence.
