Full Job Description

Join our friendly and dynamic Community Mental Health Team as a Band 5 Mental Health Practitioner, and play a vital role in supporting adults on their recovery journey. You will be part of an approachable, supportive multi-disciplinary team where your voice is valued and your development is actively encouraged. In this role, you will work closely with service users, delivering compassionate, recovery-focused care while building your confidence and skills within a well-structured, supervised environment.   • Assessments and caseload management, care planning, including risk and future support plan.
  • Delivery of interventions, - psychosocial interventions such as brief structured work.
  • Promote recovery, independence and social inclusion.
  • Risk management and safeguarding
  • Clear and accurate recording within a patient's clinical notes.
  • Collaborative working with the MDT, liaising with GP's and other agencies from voluntary or statutory sectors.
