Mental Health Practitioner Referral and Assessment Hub

This is a fantastic opportunity to be part of the Manchester Referral and Assessment Hub.

We are looking for skilled practitioners, with mental health experience, who are keen to be part of a new service in Manchester, that is central to our improved community mental health offer. You will be a practitioner who is able to complete triage and assessments at the front door service into secondary care mental health.

Successful candidates will play an important part in the in the delivery of high-quality service provision.

As part of the team, you will be empowered to shape and build the service as community transformation develops in Manchester. You will help build and maintain positive working relationships with colleagues and the wider network., As a practitioner within the RAH you will -

Provide clinical provision of rapid assessment and care/pathway planning for people referred into the RAH presenting with mental health problems.

The post holder will be expected to carry out mental health assessments, accessing all available sources of information and including, where appropriate, the views of family and carers when relevant.

To undertake and record comprehensive need/risk assessments, developing clear goals and timeframes for the onward interventions with the service users.
