Mental Health Practitioner

Are you a registered Mental Health Nurse, Social Worker or Occupational Therapist?, Mental Health Practitioner - Nurse, OT, Social Workers Working in a multi-disciplinary team and in partnership with internal and external agencies, you will;

• Provide specialist assessment and care planning in conjunction with service users and carers
• Evaluate treatment plans and undertake home treatment
• Triage and assess referrals received by the service in liaison with multi-disciplinary colleagues

The team is based at Hall Court in Telford and provides mental health assessment, treatment and support to people in Telford & Wrekin. As an integral team in the Urgent Care Pathway we work 24 hours a day, 7 days a week on a varying rostered shift pattern. Full time hours are 3 x 11 hour and 15 minute shifts with an additional 7 hour 30 minute shift per week. 33.75 hours post will work 3 x 11 hour and 15 minute shifts per week.,

  The team is based at Hall Court in Telford and provides mental health assessment, treatment and support to people in Telford & Wrekin. We work with people who are experiencing a mental health crisis, require extra support alongside their regular support network or who can have the same treatment provided at home that they would receive in a Mental Health Hospital.

In the role you will deliver a high-quality clinical service to patients and their families.
