Full Job Description

The Band 5 Community Mental Health Nurse is responsible for providing high-quality, person-centred care to adults experiencing mental health difficulties within a community setting. Working as part of a multidisciplinary team, the post holder will assess, plan, implement, and evaluate care, promoting recovery, independence, and social inclusion while ensuring the safety and wellbeing of service users.,

  • Holding a complex caseload
  • Completing medication reviews and treatment planning
  • Providing Duty and FACT cover
  • Administration of medication
  • Being a part of Multi-Disciplinary Discussions
