This innovative and exciting multi-disciplinary service is provided by Health, Social Care and Education professionals to young people aged 11-18 old years with mental health, social, emotional and behavioural difficulties, living in Surrey. We are a Centre of Clinical Excellence and awarded Outstanding by Ofsted., To be a core member of the multi-disciplinary/multi-agency Hope Service, participating in the response and provision of crisis/urgent assessments, care planning, risk assessing and developing a risk management plan and treatment plan of CYP requiring support for emotional, mental health and social care needs. Often this response will be to facilitate a diversion from T4 admission or out of area placement and allow for recovery as the C/YP remains in their home/placement/community/school., · The post holder will work closely within the Mindworks alliance during office hours, responding to crisis and urgent referrals of CYP or for crisis and urgent presentations of CYP already working with the Hope and Ext Hope Services.
· The post holder will work as an autonomous practitioner, undertaking clinical assessment of CYP who may have a complex co-morbid mental health needs and present in crisis, working alongside medical, nursing and other professional colleagues to ensure that the most effective care and treatment is provided to meet their current needs.
· Work closely with the Emergency Departments, Crisis Intervention Service, Extended Hope, CCISS, the Adult Psychiatric Liaison Service, other Mental Health Crisis/urgent Teams, On Call Duty Psychiatrist, On Call Managers, Paediatric Liaison Nurses, ensuring CYP are seen in a timely manner and risk is assessed and a management plan is developed.
· To work within a highly experienced multi-disciplinary team involved in the assessment and treatment of CYP with serious and complex mental health difficulties in crisis.
· To manage own caseload from referral to discharge/transfer, holding medium to high-risk cases and ensure timely and accurate documentation is completed.
· Ensure that outcome measures for the Hope Service are implemented and audited. Contribute to relevant audit/research and ensure that interventions on the pathway are evidence based.
· Interface with other services/agencies regarding care, treatment and discharge planning.
Band 7 Key Responsibilities (in addition to Band 6)
The post holder will work closely with the Mindworks alliance during office hours, responding to consultations or response to referrals received for possible Hope Service involvement:
· Report to and advise referrer on assessment and treatment outcomes and respond to requests for and provide formal reports for other agencies.
· Making independent decisions regarding risks and needs of young people with mental health concerns.
· Represent the Hope Service at professional/network meetings to offer advice/consultation on complex cases who are in crisis.
· Collaborate on developing systems to monitor and evaluate the service
· Supervise other junior members of the team and contribute to their appraisal process.
· To undertake relevant CPD to improve assessment and treatment skills.
· Be competent in conducting assessments in all settings.
· Deputies for the Clinical Lead Nurse where required

This is a development role. We are seeking a Band 6 to 7 Mental Health Nurse Practitioner, to work at the Hope service. The Mental Health Nurse Practitioner will work with young people with complex mental health, behavioural and social care needs. They will provide mental health assessments focusing on risk management, needs of the young person and provide the appropriate intervention and/or sign posting for the young person. To offer consultation to the team on mental health related issues. We are looking for a dynamic and motivated staff member with experience in working with young people with complex needs and support their colleagues in both Epsom and Guildford.
