Full Job Description
This role will work under the supervision of the Memory Service Clinical Nurse Specialist offering advice and support to clients who are experiencing suspected Dementia/Mild Cognitive Impairment, as well as their carers.
Take responsibility for a clinical caseload providing care and treatment to clients referred into the Memory Service.
Foster robust working relationships with all partners in particular Local Authority and Primary Care.
Contribute to the continuing improvement and development of dementia care services.
Promote physical and mental health wellbeing, emphasising living well with dementia.
Be an advocate for clients living with dementia and their carers, and signpost as necessary to statutory and voluntary services.
Complete assessments and follow up reviews for people within the Memory Service.
Welsh Skills Essential: This post is advertised as Welsh skills are essential. This means Speaking & Listening at Level 3 (equivalent to CEFR B2) or above. We don't need fluency. We're looking for a dedicated Memory Service Practitioner to provide compassionate assessment, support and guidance to clients and their carers across a wide range of multi-disciplinary services within CTMUHB.
In this rewarding role, you will deliver holistic, person-centred assessments while working collaboratively with colleagues across multi-disciplinary and multi-agency teams. Your expertise will help shape tailored support that meets the full spectrum of clients' needs.
A key focus of the role is empowering individuals to maintain the highest possible level of physical and mental wellbeing. You will play a vital part in helping clients remain safely and independently at home for as long as appropriate, promoting confidence, resilience and quality of life.
