Our Acute Medicine Team includes experienced Pharmacists and Medicines Management Technicians who work collaboratively to deliver high-quality care. You will work closely with a range of healthcare professionals to provide ward-based medicines management and optimisation. This includes medicines reconciliation, patient counselling, and supporting the safe, timely preparation of discharge medicines.
