Full Job Description

We are currently recruiting for a Medicines Management Care Co-ordinator to join our Pharmacy Team based at our Northbourne site. This is a full-time post working 37 hours over 5 days, however 4 days would be considered.

As a Medicines Management Care Co-ordinator you will be plays a key role in supporting the safe, effective, and efficient management of patients' medicines within the GP practice. Working closely with GPs, Clinical Pharmacists, Pharmacy Technicians, nursing staff, community pharmacies, and patients, you will co-ordinate medication-related administrative and clinical support activities., The role focuses on improving patient care, supporting medicines optimisation, reducing medicines waste, ensuring timely medication reviews, and helping patients access the most appropriate pharmaceutical services.
