MEAM Coordinator

MEAM works with local areas to design and deliver better coordinated services for people experiencing multiple disadvantage, including homelessness, substance use, contact with the criminal justice system, and mental health challenges As the MEAM Coordinator for Thanet, you will lead the coordination of this innovative approach - working across health, social care, housing, criminal justice, and the voluntary sector to break down barriers, influence change, and co-produce new ways of working with people who know the system best: those who have lived it. The MEAM Coordinator will engage with all relevant bodies and build close working relationships across sectors. Hence, excellent relationships building and partnership working is essential for this role., As the MEAM Coordinator, you will:

• Lead the Thanet MEAM pilot, building strong relationships across public, voluntary, and community sectors.
• Co-produce service solutions with people who have lived experience of multiple disadvantage.
• Coordinate multi-agency support for individuals in the first Thanet MEAN cohort using a trauma-informed, strengths-based approach.
• Identify and escalate barriers within the system that prevent equitable access to services.
• Gather and analyse data to evidence impact, inform commissioning, and influence policy.
• Facilitate learning and reflective spaces for frontline teams to embed MEAM principles.

  Significant experience of working directly with individuals with complex life experiences.
• Proven ability to coordinate and deliver personalised case management in a multi-agency setting.
• Experience facilitating reflective practice or professional learning.
• Demonstrable experience of negotiating and influencing at both operational and strategic levels.
• Experience in collecting, analysing, and reporting on quantitative and qualitative data.
• Experience in co-production with people with lived / living experience., Track record of leading service improvement or contributing to systems change.
• Experience supporting governance structures or project boards.
• Previous experience delivering training or facilitating networks.
• Lived experience of multiple disadvantage., In depth knowledge of support systems for people facing multiple disadvantage (e.g. homelessness, mental health, substance use, offending).
• Strong understanding of trauma-informed, strengths-based, and person-centered approaches.
• Knowledge of safeguarding principles and legal responsibilities relating to adults and children.
• Familiarity with risk assessment and management processes in multi-agency contexts.
• Knowledge of GDPR and data management best practices., Relevant professional qualification (e.g. social work, health, housing, criminal justice).
• Knowledge of MEAM (Making Every Adult Matter) approach and its national framework.
• Understanding of local governance structures and commissioning processes.
• Knowledge of Thanet's location and challenges.
