Full Job Description

We are looking for a dedicated MBT Practitioner to join our dynamic team and provide specialist, evidence-based treatment to patients within the City and Hackney community personality disorder service, which is part of the broader Complex Emotional Needs pathway. As a member of a multidisciplinary team, the successful candidate will be responsible for conducting individual assessments and delivering identified clinical interventions, both in group settings and one-on-one. This role involves offering direct care to service users facing complex emotional challenges, often associated with personality disorders and related difficulties. · Undertake assessment of emotional and psychological needs of patients referred to the service. · Formulate and implement appropriate treatment plans and provide mentalisation-based therapy at the same time as acting as lead professional to those patients working closely with other members of the team within and outside of the service. · Liaise with other professionals to contribute to the establishment of continuity of care across services. · To manage a caseload of patients, in close collaboration with professional guidelines and the overall framework of the teams' policies and procedures. · To ensure that continuity of care is maintained and that appropriate and timely plans are made for the discharge of patients from the inpatient facilities. · To conduct assessments and assign patient to care clusters within the framework at first assessment and thereafter at reviews or of significant changes in presentation.
