Macmillan Patient Pathway Facilitator

An exciting opportunity for a Band 4 position to join the existing Patient Pathway Facilitator (PPF) team, supporting Cancer Services. We are seeking a motivated, dynamic, proactive individual to join our team of administration staff to support the Rarer and Urology Clinical Specialist Nurses.

The post holder will be required to work autonomously as well as part of the immediate and larger team with clear objectives on supporting the nurse specialists to deliver key priorities in cancer including Holistic Needs Assessment and End of Treatment Summaries and Stratified Follow Up., We are seeking an individual who has a clear vision to meet patient and Trust needs ensuring clinical governance is a priority alongside patient safety.

To achieve these goals, the successful candidate will need to have previous administrative experience including working with databases, spread sheets and managing software systems to ensure the patient pathway is fully supported. An aptitude for problem solving and an eye for detail are crucial.
