Full Job Description

At Sandwell and West Birmingham NHS trust we are very proud of our specialist palliative care and End of life care Co-ordination Hub. The CQC awarded us excellent for our service in 2017. The team consists of specialist Palliative consultants, palliative care clinical nurse specialists for the Acute and community, end of life care facilitators, an Urgent response team and Specialist Macmillan Occupational Therapists & Physiotherapist. The Connected Palliative Care Hub provides a 24/7 service to patients who are in hospital and the community in Sandwell and West Birmingham. The Macmillan Therapy Team offers services to palliative patients in the Acute Hospital, Day Hospice and in their own home., The successful candidate will be Flexible to be able to work within the Acute Hospital or Community setting. Be able to navigate the faced paced nature offacilitating Fast track discharges home for patients within the Acute Hospital setting, where home is their preferred place of care. And embody the respect required to enter a patients own home. Carrying out Holistic Needs Assessments on palliative patients establishing client centred goals that patients wish to achieve along their journey taking into consideration limitations surrounding their disease trajectory. Using therapeutic activities to provide advice and techniques that empower patients' self-management of symptomatic relief. Supporting these patients with advanced care planning and promoting quality of life up to and until end of life.
