Full Job Description
An exciting opportunity has arisen for an enthusiastic and dynamic individual to join our team as a Lung Cancer Patient Navigator.
You will support the Respiratory service to facilitate the smooth running of the patient journey, by ensuring that the lung cancer pathway is coordinated, streamlined and all processes revolve around the patient and their individual needs.
The role of the lung cancer patient navigator is to provide specialist knowledge for the pathway of lung cancer patients . The role will facilitate the smooth running of the patient journey by ensuring that the pathway is coordinated, streamlined and all processes revolve around the patient and their individual needs.
This is a 12 month Fixed post due to funding.
If successful you will work directly and indirectly with patients and their carers, and be responsible for capturing information about patients, ensuring up-to-date relevant information is recorded on the patient record and Trust systems.
You will have a good knowledge and understanding of patient complete pathways. As a key requirement of this role, the post holder will have excellent communication and organisational skills, together with a high attention to detail and levels of accuracy.
