Working within the Lotus Team, they coordinate personalised, trauma-informed care for women experiencing vulnerabilities including domestic abuse, mental ill health, substance misuse, homelessness, exploitation, social deprivation and safeguarding concerns.
Key responsibilities include managing a caseload of vulnerable women; undertaking holistic assessments and developing individualised care plans; coordinating care with maternity services and partner agencies including social care, housing, perinatal mental health, substance misuse and domestic abuse services; identifying and escalating safeguarding concerns; and attending multi-agency meetings including Child Protection Conferences, Strategy Meetings, MARACs and pre-birth planning meetings.
The role also involves advocating for women and families, promoting engagement with maternity services, delivering antenatal and postnatal care, supporting infant feeding, maintaining accurate records, contributing to audit and service improvement, and promoting safeguarding and trauma-informed practice.
