Long Term Conditions Nurse

We have an exciting opportunity for a Long Term Conditions Nurse to join our Primary Care team at HMP Hewell, a category B Prison located in Redditch. The successful candidate would need strong long term conditions experience. Opportunities for prescribing and health assessment courses are on offer, if you are interested. Hours 37.5 - 3 days a week 5 day week 8:30am-4:30pm, As a Long Term Conditions Nurse, your responsibilities will vary. You will

• be responsible for ensuring a comprehensive handover of services when patients are transferring across wings or custodial care settings.
• identify immediate health needs including, any external services or prescribing requirements that prisoners may have received e.g. prior to remand or repatriation onto a wing
• facilitate arrangements between agencies involved in delivering a complex care package to ensure effective collaboration and stability for the patient in delivery of that care. This requires close joint working with other in-house healthcare services and departments within the prison such as Pharmacy, Education, Offender Management, Substance Misuse and Physical Education, etc.
• Provide proactive intervention to urgently address improvement recommendations from CQC and HMPP inspections
• Deliver continuity of care across health and social care agencies from courts, to custody and community whilst improving the communication between the above.

  Registered Nurse- General with relevant NMC Registration
• Advanced Nurse Prescriber/ Practitioner
• Educated to degree level

The salary for this role ranges between £43,609 - £49,175 per annum- depending on experience. How will we support you?
