Full Job Description

This is a fantastic opportunity to join an exciting new development in our mental health service, in a CQC rated Outstanding Trust. As part of our community transformation, aimed at more integrated service, we are creating a new Front Door Service.

The 'front door' will be the entry point to ELFT mental health services in City & Hackney. It will be a single front door for urgent and non-urgent referrals.

You will lead a well-resourced MDT including resident doctor, senior practitioners, psychology professionals, working at the interface between primary and secondary care mental health services.

The service base is very central to the borough with good bus and overground connections.

Main duties of the job

There is 0.6 FTE post made available due to service redesign.

This post will provide joint clinical leadership with the senior Psychologist. They will provide a link to the Neighbourhood/Quadrant psychiatrists and provide medical input and oversight to the Front Door.

The main duties of the post consist of providing consultant leadership, supervision and support to their team. The post-holder will participate, directly and indirectly, in the assessment of patients referred to the service and in drawing up coproduced formulations which will address the patients' psychiatric, physical, psychological and social needs, with the input of the whole multidisciplinary team. When appropriate, the post holder will assess the patient and co-produce a care plan before discharging back to primary care. Good communication will be undertaken with the patient's GP and workers in the team.

The post holder will also participate in daily referrals meetings, where they will provide senior clinical leadership, alongside the psychology professional lead, and operational lead, supporting the team triaging referrals, making decisions about allocation of assessors, or whether the referral should be passed to another service in or outside of ELFT.
