Cambridgeshire Community Services NHS Trust is seeking a dynamic nurse for a shared post between the Integrated Discharge Team at Luton & Dunstable Hospital and the Rapid Response Team in the community. The role focuses on autonomous practice, complex clinical decisions, and leading others to deliver patient-centred care with strong discharge planning knowledge.
Successful candidates will contribute to preventing admissions and enabling timely, safe discharges, with shift and weekend work.
