Full Job Description

Are you an enthusiastic individual who is committed to making a difference to the lives of people with learning disabilities every day? Have you got experience of implementing reasonable adjustments to ensure people with learning disabilities have an equal quality of care and experience? Do you enjoy spending time with patients and supporting colleagues? We are looking for support workers to join our friendly Learning Disability Liaison teams in both Yeovil Hospital and Musgrove Park Hospital. You will support people with learning disabilities, and the people who matter to them, through acute admissions, outpatient appointments, elective procedures or scanning. Your role will be to engage and support patients to access the hospital, providing reassurance, stimulation and activity. You will understand the needs of individuals and what reasonable adjustments they might need, and assist acute colleagues to provide the best possible care. This is an exciting time for the Learning Disability Liaison Service. With our expansion we have new opportunities to shape the future of of the support we provide and make a meaningful difference for patients with learning disabilities. You will be supported by a small team of experienced and qualified staff who are committed and passionate about the work they do. "I've learned that people will forget what you said, people will forget what you did, but people will never forget how you made them feel."- Maya Angelou, You will be asked to support people with learning disabilities to access the acute hospital setting, liaising with the people who matter to them and with acute colleagues to ensure reasonable adjustments are made and the best possible care is provided. You might sit with a person on an acute ward and speak to them or help them with an activity to engage and distract them. You might attend an outpatient appointment and offer reassurance to a person who is worried about coming to hospital. You might go to Day Surgery and support acute staff to meet the person's needs. You might be asked to speak to staff in a particular department about the importance of using Health Passports and how they can help people with learning disabilities communicate their needs. The job is a varied as the patients who you will meet, no two days are the same.
