Full Job Description

The purpose of the job is to work closely with the clinical and safeguarding teams in supporting all activities necessary to ensure that the Trust meets its responsibilities to patients with dementia and learning disability. This work will be promoting best practice, implementing standards and monitoring the process in line with both national and local multi-agency guidelines.

You will support in the care of complex patients in accessing services across all our network sites as well as he successful transfer of care following the completion of care, supporting clinical staff by being the point of liaison between the community/home and the service keeping the patient at the centre of their care.

You will be expected to work without direct supervision to provide expert advice and support to a variety of staff on complex decisions around learning disability, autism and dementia. Identify barriers to accessing healthcare for people with a learning disability and reasonable adjustments are in place to ensure equality of access.

You will have significant experience in relation to learning disabilities and dementia and have knowledge of relevant legislation such as the Care Act 2014, the Mental Capacity Act 2005 and Deprivation of Liberty Safeguards 2009/ Liberty Protection Safeguards. You will contribute to the Safeguarding Team's ability to ensure that the Trust's compliance and monitoring of the statutory responsibilities., In collaboration with the Lead for the safeguarding team /Named Nurse for Adult Safeguarding and the multi-disciplinary team you will be responsible for the delivery of safe, effective care leading to an excellent experience for patients and carers by;

o Being responsible for ensuring high quality, evidence-based patient centred care is delivered and providing assurance of this whilst delivering the Trusts core objectives and strategies.

o Developing and delivering continuous improvement initiatives to ensure that the service meets all quality indicator targets, key performance indicators and quality and safety standards and initiatives relevant to the service.

o Deliver the learning requirements to staff in the trust through planned education activities in line with the learning disability and autism code of practice

o Reviewing and implementing the management of patient pathways to improve the quality of care and efficiency in the wards/departments, ensuring the patients experience is positive while under our care.

o Leading on the trust's clinical practice standards, clinical improvements and/or strategies for the delivery of excellent care for patients with learning disability, autism or dementia and subsequent training and education requirements
