An exciting opportunity has arisen for an experienced and motivated Complex Case Manager to join our Continuing Healthcare (CHC) Team. This rewarding role supports adults with complex health needs requiring coordinated, person-centred care and support. As a Complex Case Manager, you will ensure individuals receive the right care, at the right time, in the most appropriate setting, achieving the best possible outcomes for patients, families and carers. Working across health and social care systems, you will undertake assessments, coordinate complex care packages and work collaboratively with a range of stakeholders to deliver high-quality Continuing Healthcare services., As a Complex Case Manager, you will:

  • Undertake comprehensive Continuing Healthcare assessments and reviews to determine eligibility and ensure care remains appropriate to need.
  • Coordinate and manage complex packages of care, ensuring services are safe, effective and focused on achieving positive outcomes.
  • Develop person-centred care plans in partnership with service users, families, carers and multidisciplinary teams.
  • Act as an advocate for individuals, ensuring their views and preferences remain central to decision-making.
  • Work collaboratively with health, social care and provider organisations to deliver integrated and coordinated services.
  • Lead and participate in multidisciplinary meetings, case conferences and care reviews.
  • Identify, assess and manage risks, ensuring safeguarding responsibilities are met and appropriate action is taken.
  • Monitor the effectiveness of care packages and recommend changes where needs evolve.
  • Contribute to service improvement initiatives and support the ongoing development of Continuing Healthcare services.

    We are seeking a Registered Nurse with current NMC registration and degree-level education, significant post-registration experience working with individuals with complex health needs, and experience undertaking holistic assessments within multidisciplinary and multi-agency settings. You will have a sound understanding of the National Framework for NHS Continuing Healthcare, NHS-funded Nursing Care, the Mental Capacity Act, Deprivation of Liberty Safeguards and safeguarding principles. Strong clinical leadership, communication, negotiation and organisational skills are essential, together with the ability to manage complex situations, work independently and remain committed to improving outcomes through person-centred care.
