Full Job Description
An exciting opportunity has arisen for an enthusiastic and experienced Learning Disabilities Clinical Assessor to join the Continuing Healthcare (CHC) Team within North West London. This post is ideal for a Registered Learning Disabilities Nurse (RNLD) or suitably experienced Allied Health Professional who is passionate about improving outcomes for people with learning disabilities and complex health needs. Working within a specialist multidisciplinary team, you will assess, monitor and review individuals in line with the National Framework for NHS Continuing Healthcare. You will work closely with service users, families, health and social care professionals, provider organisations and local authorities to ensure individuals receive person-centred, safe and effective care that meets their assessed needs., As a Learning Disabilities Clinical Assessor, you will:
- Undertake NHS Continuing Healthcare assessments, reviews and monitoring visits for individuals with learning disabilities and complex health needs.
- Apply the National Framework for NHS Continuing Healthcare and associated legislation to support robust, evidence-based decision-making.
- Work collaboratively with multidisciplinary teams, provider organisations, local authorities, service users and families to achieve positive outcomes.
- Gather, analyse and present complex information from multiple sources to inform assessments and reviews.
- Support person-centred care planning and ensure services remain responsive to changing needs.
- Provide specialist advice and guidance relating to NHS Continuing Healthcare processes and eligibility.
- Contribute to safeguarding processes and risk management activities.
- Maintain accurate clinical records and documentation in accordance with professional and organisational standards.
- Participate in service improvement initiatives and support the delivery of high-quality Continuing Healthcare services.
- Effectively manage a varied workload, balancing competing priorities while working independently and as part of a wider team.
