Full Job Description

The Lambeth Living Well Network Alliance supports people in Lambeth experiencing mental illness. Thames Reach works in partnership with South London and Maudsley NHS Foundation Trust, Lambeth NHS Clinical Commissioning Group, Lambeth Council and Certitude to deliver mental health services as part of the innovative transformation of mental health services in the borough.

We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.

You will:

.Support clients living in the community using a mix of brief interventions and longer-term support
.Work as part of a multi-disciplinary team, community psychiatric nurses, social workers, occupational therapists, peer mentors and consultants
.Manage a caseload of people with complex mental health needs
.Work autonomously and as part of a team
.Accurately record work to enable us to demonstrate our outcomes
.Contribute to the meeting of the targets in the areas of Education, Training and Employment / move on of the service users, maximising service user involvement /and other Key Performance Indicators

You will be:
