Lead Practice Nurse

The post holder will have responsibility for providing managerial and professional leadership to the nursing and nursing support team. They will work closely with the Practice Manager to deliver, plan and future proof services, leading the team to provide an exemplary clinical service whilst ensuring practice targets are maintained and achieved to optimal levels. Working as part of a multi-disciplinary team the post holder will be accountable for the delivery and standards to ensure safe, effective, person centered care. The post holder will have broad knowledge and experience of treating and working with patients and multi-disciplinary teams to maximise health outcomes within all chronic disease areas. The post holder will be responsible for the management of Infection Control.

We are looking for an enthusiastic, committed, dynamic and forward-thinking individual to work collaboratively across the multi-disciplinary clinical and management team to support the delivery of outstanding care to the patient population, including acute and chronic condition management. The successful candidate will have the ability to manage, motivate, train and support the Nursing Team consisting of 2 Practice Nurses and 2 HCAs The successful applicant should be an experienced Nurse with proven ability of the above. They will have the vision and ability to be able to drive change and enhance service development to provide optimal primary care services for the benefit of the patient population.
