Full Job Description

We're looking for a dynamic, forward-thinking pharmacist to lead and shape the pharmacy service at the PACH (The Haslemere Proactive Care Hub) launched by the Royal Surrey NHS Foundation Trust. The hub is part of a £1 million investment to bring hospital-grade, community-based care closer to home This is a rare chance to take on a high-profile clinical leadership role during the development of this new integrated healthcare facility., You will be working as an Independent Prescriber within a multidisciplinary team. You will deliver specialist patient care mainly in an out-patient setting
Leading on safe, cost-effective prescribing across a variety of patients with long -term conditions, including monitoring drug usage, expenditure, and biosimilar switching
Developing and implementing prescribing guidelines, policies, and protocols for the department
Representing pharmacy at governance and service / finance meetings
Acting as a clinical role model supporting, mentoring, and educating junior pharmacists, rotational staff, and other healthcare professionals
Driving service development and innovation, with the opportunity to participate in research and audit, The Lead Pharmacist for the Long-term Conditions Hub (PACH) provides clinical and professional leadership for pharmacy services supporting patients with long-term conditions across community and outpatient pathways. Working as an Independent Prescriber within a multidisciplinary team, the post holder will deliver specialist patient care, lead medicines optimisation initiatives, and support improved health outcomes through proactive management of complex long-term conditions.
The role is responsible for developing and evaluating pharmacy services within the hub, promoting safe and effective use of medicines, reducing avoidable hospital admissions, and ensuring seamless care across primary, community and secondary care settings. The post holder will contribute to clinical governance, quality improvement, education, research, and financial stewardship, while providing leadership and support to pharmacy staff and wider healthcare professionals.
As a senior clinical leader, the post holder will work collaboratively with system partners to develop innovative, patient-centred services that enable people with long-term conditions to receive high-quality care closer to home.
