We are delighted to launch a brand-new Band 7 Lead Nurse for Transition to Adulthood role, offering a unique opportunity to shape and lead the development of transition services across Mid Yorkshire Teaching NHS Trust. This is an exciting time to join us. As the first postholder in this role, you will have the opportunity to design, influence and embed a high-quality transition pathway that supports young people as they move from children's to adult services. Working across multiple specialties and professional teams, you will ensure young people receive seamless, safe and person-centred care throughout their journey.,
- Lead the development and implementation of the Trust's transition strategy.
- Champion the needs and voices of young people and their families.
- Work collaboratively with paediatric, adult and community services to improve patient experience and outcomes.
- Drive quality improvement, innovation and service transformation.
- Provide expert leadership, education and support to clinical teams.
- Develop strong partnerships with internal and external stakeholders to enhance transition pathways.
