Full Job Description
We are excited to be able to recruit an experienced nurse to lead the Royal London Hospital's Frailty Team. The post holder will lead a team including Frailty Practitioners (nurses and therapists), Frailty Clinical Nurse Specialists and Frailty Support Workers in the delivery of both the Royal London Hospital's Acute Frailty Service and the Tower Hamlets Frailty Virtual Ward, providing day-to-day line management and support., This post will be of potential interest to staff who are working at Band 7 or 8, who have extensive experience of working with older people, a track record of effective team leadership and service development, and who wish to progress and apply their expertise in a stimulating clinical environment.
The successful candidate will have a work plan which is at least 80% based in clinical practice as an expert practitioner and clinical leader in the frailty service delivering person-centred care, frailty screening and comprehensive assessment for frail older people across both the acute frailty pathway at the Royal London Hospital and the Frailty Virtual Ward; and supporting their team in delivering these aspects of care and support.
The post holder will be expected to use their experience and knowledge to support the ongoing service development, transformation work and delivery of frailty services, both within the Royal London Hospital and for the Tower Hamlets Frailty Virtual Ward, including managing data collection and service evaluation
The post holder will use their skills and expertise to contribute to frailty teaching and training across the Trust. The post holder will contribute to frailty network working across the Trust, across NEL and across London/nationally.
The post holder will deputise for the Senior Nurse of Acute Medicine and Older People Services when required, ensuring, safety, quality and performance is maintained across the directorate.
