Are you an experienced and passionate transplant nurse ready to take the lead in our Living Donor programme? This is an exciting opportunity to join the Oxford Transplant Centre as our Lead Living Donor Nurse Specialist, playing a pivotal role in supporting and guiding living kidney and uterus donors through their transplant journey. You will coordinate and manage the assessment, work-up, and care of individuals considering living donation. You will become a trusted advocate for donors and their families, providing expert clinical support while addressing the psychological, emotional, and social aspects of donation. Working alongside a highly skilled multidisciplinary team, you will help ensure that every donor receives compassionate, safe, and personalised care from initial referral through to long-term follow-up. This dynamic role offers the opportunity to influence and shape a growing and developing service. You will provide professional leadership, drive service improvements, support innovation, and contribute to the ongoing development of nursing practice within transplantation. Working closely with the Lead Living Donor Transplant Consultant and wider transplant team, you will ensure the highest standards of care are delivered to our donors throughout their inpatient and outpatient journey., The Lead Living Donor Nurse Specialist role is to lead on the coordination and management of patients within the Oxford Transplant Centre that are undergoing assessment and work up for a living kidney or uterus donation. They will also work closely with the MDT during the patient's inpatient stay and overseeing their follow up post-transplant. The role involves conducting patient consultations, addressing clinical, psychological and social needs under the supervision of the Lead Living Donor Transplant Consultant. The post holder also works within the wider Transplant Specialist Nurse team providing on call transplant recipient coordination 8am-8pm and at weekends (occasionally 24 hours). In periods of operational pressure, the postholder will be expected to provide cross-cover locally within the Transplant specialist nursing team and wider Directorate as required. The post holder will provide excellent professional and clinical leadership within Living Donor transplant promoting the effective delivery of a high standard of nursing care for all living donor patients and contributing to the development of the service, practice and colleagues., · Demonstrate clinical expertise in the delivery of direct nursing care: co-ordinating and monitoring the care of living donor patients, acting as a role model to the nursing and wider MDT and demonstrating partnership with patients and their carer's. · Work autonomously as well as within a multidisciplinary team to provide assessment of patients who come forward for living donation. This involves requesting and the follow up of blood tests and investigations, history taking and physical assessment. · Liaise with referral centres to ensure patients work up pathway is seamless and all relevant clinicians are communicated to effectively. · Plan and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimize health outcomes and resource use, providing direct support to patients. · Ensure the effective delivery of a high standard of nursing care for all patients within the nursing team by providing clinical leadership, direction, supervision and maintaining effective systems of work.

  • Provide triage and management of acute patient enquiries via telephone advice line.
  • Practice a high standard of communication including highly sensitive and complex information to develop and sustain partnership working with individuals, groups, communities and agencies. Utilise evidence based information through a variety of communication media.
  • Promote a high standard of specialist nursing by initiating and co-ordinating the assessment, planning, delivery and evaluation of the holistic needs of patients/families through evidence based practice following agreed policies, protocols and guidelines.
  • · Participate in the transplant recipient co-ordination on-call, including working weekends and bank holidays. · Adhere to the NMC Code of Conduct at all times and practice within the NMC Scope of Professional Practice. Management · Monitor and plan in advance the workload of the living donor nursing team, ensuring that it is adequately resourced; co-ordinate and ensure allocation of patients. · Proactively recognise and respond to factors which impinge upon staff morale and standards of care, liaising with the Deputy Matron as appropriate. · Facilitate regular and effective appraisals for the band 6 within the team and support appropriate training and development opportunities in line with role requirements, departmental objectives. · Collaborate with the multidisciplinary team to develop a shared vision of the service. · Network locally, regionally, nationally and internationally and seek opportunities to develop the programme. · Act as a role model and resource in expert clinical evidence based practice. · Work collaboratively with colleagues to develop effective documentation, pathways, protocols and guidelines for care. Demonstrate a commitment to share expertise and disseminate information. · Participate in implementing national standards and frameworks at a local level. · Use effective change management skills to initiate and implement service and practice development, making best use of resources to improve practice and health outcomes. · Have an understanding of budgetary constraints and may be required to manage budgets. · Use effective change management skills to initiate and implement service and practice development, making the best use of resources to improve practice and health outcomes. · Have active involvement/lead where appropriate in the recruitment process including the selection and appointment of new staff · Maintain involvement and active input in multidisciplinary meetings and appropriate local bodies / forums to contribute to practice and service development Education/Training: · Maintain, advance and develop personal theoretical knowledge, high standards of clinical competence and professional development through ongoing self-education and attendance at relevant courses of study. · Develop and maintain a peer network of support, information and learning with other nurse specialists at a local and National level. · Maintain own professional portfolio. · Contribute to the development of specialist nursing knowledge and skills to enhance and extend the practice of other nurses and health care professionals within the service. · Identify and address the educational needs of patients, families, carers and staff involved in the delivery of this service. · Contribute to the teaching and support of students from the multi professional team within relevant educational programmes. · Provide an appropriate learning environment, including mentorship, coaching and preceptorship. · Organise and contribute to relevant educational programmes and in house study days. · Develop and maintain a peer network of support, information and learning with other nurse specialists at a local and National level. Improving quality and developing practice:
  • Coordinate and participate in ongoing service review and evaluation, including monitoring of patient experience.
  • Participate in research / audit to generate new evidence in area of speciality.
  • Evaluate clinical practice in relation to its evidence base and clinical effectiveness.
  • Identify areas of risk and poor quality, and address these through appropriate governance structures and forums.
  • Participate and actively seek peer review in practice.
  • Have responsibility to ensure annual appraisal, agree objectives which reflect local and national service needs and develop personal development plans.
  • Develop personal and professional knowledge through critical self-appraisal, clinical supervision and networking with other health care professionals locally, nationally and internationally.
  • Advocate and contribute to a working environment that promotes continuous learning and development, evidence based practice and succession planning, as well as identifying learning opportunities at local and national levels.
  • Have a high level of communication skills to enable the participation in the dissemination of practice through conferences and professional publications.
  • Ensure statutory and mandatory training updates, as well as role /speciality updates are completed and necessary records maintained.

    Minimum 3 years Transplant experience, You must have appropriate UK professional registration.
