Lead Clinical Practitioner -Burren

The Lead Clinical Practitioner (LCP) is a senior leadership role within Severnside Integrated Urgent Care (IUC), providing advanced clinical practice across The Burren, Out of Hours, Clinical Assessment Service (CAS), Clinical Medication Delivery Unit (CMDU) and ACE services. The role combines clinical duties with leadership responsibilities, including supervision, mentoring, clinical governance, audit, service improvement and patient safety.

Within The Burren, the post holder will work as an autonomous prescribing clinician, delivering primary medical care to patients discharged from secondary care who require rehabilitation and ongoing clinical support. The role requires advanced assessment, clinical decision-making, risk management and chronic disease management skills, supported by the Nursing and AHP leadership team and primary care GPs.

The post holder will act as a key member of the multidisciplinary team, participating in ward rounds and managing routine and urgent patient needs. Working closely with nurses, AHPs, GPs, therapists, social care and operational teams, the LCP will support integrated care, improve patient outcomes and ensure patients receive the right care at the right time. The post includes 16.75 clinical hours and 6 leadership hours per week.

Main duties of the job

ClinicaloAssess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, complex needs, acute and chronic conditions for both physical, mental health and psycho-social needs.oClinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordinglyoDiagnose and manage patient presentations, integrating both drug and non-drug-based treatment methods into a management plan

CMDU OnlyoThis is a service for North Somerset prescribing antivirals for Covid. This service is run during set hours and has one allocated clinician.

CommunicationoEffective communication as part of the Nursing & AHP Leadership Team - providing representation across IUC, ensuring partnership working and working with a variety of stakeholders.oUtilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatmentoCommunicate with and support patients who are receiving 'bad news'oCommunicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background, languages, and preferred ways of communicating.
