The Transformation Team currently has a fixed term vacancy for 12 months for a Lead Change Specialist to manage a range of change projects around planned care. The dynamic team support a wide variety of projects and programmes of work taking a pathway approach, therefore working in close partnership with colleagues across the health and social care system. Applicants should have an interest in planned care and sustainable service delivery along with experience in managing complex change across a health and care system., The successful candidate will report to the Head of Transformation and will play a key role in ensuring successful delivery of the local priorities from the Planned Care Improvement Programme and local implementation of Accelerated National Innovation Adoption projects. The post holder will have experience of leading on the delivery of complex programmes where redesign of systems and processes, and cultural and behavioral change are required for success. As from 1/4/26, the Agenda for Change full-time working week will be reducing from 37 to 36 hours per week. Part time hours will be reduced pro-rata. However, there will be a corresponding increase in the hourly rate so that pay will not be affected.
