Full Job Description

Working under the supervision of a registered clinician, you will help translate clinical formulation into day to day practice, strengthen staff capability, and promote a culture of learning, safety, and human rights.,

  • Positive Monitoring: Conduct structured observations, provide immediate feedback, and lead BSP audits to ensure high quality, compliant care.
  • Staff Coaching: Deliver structured coaching, support PBS and RRN accredited training, and promote psychologically safe team cultures.
  • Restrictive Practice Reduction: Lead blanket restriction reviews, oversee restrictive intervention governance, and ensure least restrictive practice.
  • Crisis Support: Provide intensive onsite support during periods of acute behavioural disturbance and support urgent BSP revisions.
  • Post Incident Support: Facilitate debriefs, ensure learning is embedded, and support staff and service users following incidents, Working under the supervision of a registered clinician, you will help translate clinical formulation into daytoday practice, strengthen staff capability, and promote a culture of learning, safety, and human rights., 1. Positive Monitoring and Quality Assurance
  • Direct Observation: Conduct regular in vivo (structured) observations of staff to evaluate the effectiveness and implementation of Behaviour Support Plans (BSPs) to a rota agreed with operations directors and other senior staff.
  • Feedback Loops: Provide immediate, constructive feedback and positive reinforcement to staff teams to improve plan delivery and therapeutic engagement. To deliver coaching catch ups to staff teams, prioritising according to clinical acuity and risk, to ensure that IRs Care Groups commitment to team support and psychological safety is upheld
  • Audit Management: Oversee the quarterly audit of all BSPs using the BSP Quality Audit Tool, identifying areas of non-compliance related to physical risk, medication use, or statutory requirements. To produce an improvement plan for each service and to monitor implementation. To produce a report for the board on any deviation on the critical safety audit items to ensure board visibility of service non compliance or risks.
  • Governance Reporting: Collate and present "Positive Monitoring" data for monthly Multidisciplinary Team (MDT) meetings and local governance forums.
  • Rapid Culture Reviews to use the results of relevant staff feedback, incident information, whistleblowing information and other sources of intelligence to support the board of directors and the ethics and safeguarding lead to deploy rapid culture reviews to support the safety and quality of Iris Care Groups services
  • 2. Staff Coaching and Culture Development
  • Practice Leadership: Act as a visible practice leader on shifts, guiding, supporting, and directing prescribed interventions. Ensure that there is an identified lead PBS worker for each person we support, in line with Iris Care Group policy requirements and safety workflow.
  • Culture Feedback: Implement evidence-based methods for measuring and reporting on staff culture, identifying "closed cultures" that may minimize rights-based approaches.
  • Coaching Catch-ups: Facilitate regular structured one-to-one coaching sessions with support staff to ensure they understand how to use evidence-based approaches and avoid clinical deterioration in people with complex needs.
  • Training Liaison: Support the delivery of Positive Behaviour Support (PBS) and Restraint Reduction Network (RRN) accredited physical intervention training. To offer when required Network Training to teams working with new people we support or where additional training and reformulation is required for urgent care modifications
  • Safety 1 and Safety 2 : To champion the See it say it send it approach to safe care at all times. To promote and support learning from error in an environment of psychological safety and the visible championing of all practice that promotes safety.
  • 3. Restrictive Practice Reduction
  • Blanket Restriction Review: Lead the "4 Rs" (Rules, Reason, Rights, Review) approach to identify and eliminate unauthorized blanket restrictions.
  • Restrictive Intervention Reviews: Coordinate and/or conduct individual and contained environment reviews following high-frequency or high-duration restraint episodes.
  • Adaptive Hold Governance: Oversee the referral and training process for Non-Standard Holds, ensuring they are only used as a last resort under strict MDT and best-interests frameworks.
  • 4. Crisis Support and Admission Prevention
  • Urgent BSP Revision: To support and monitor the Immediately withdrawal and revision of BSPs following incidents involving high risk, abnormal physical observations, or psychological deterioration, adhering to Iris Care Group policy timelines
  • Admission Prevention: Provide intensive on-site support during periods of severe disturbance to stabilise environments and prevent the need for hospital recall or emergency admission.
  • Engagement Monitoring: Act as a senior point of escalation for changes in Observation Levels, ensuring the least intrusive level is used at all times.
  • Physical Intervention team interface: to support compliance with the process for PI team referral in cases where adapted holds are required. To ensure compliance with Iris Care Groups timelines for BSP review following the prescription and training of adapted holds.
  • 5. Post-Incident Support and Debriefing
  • 3Rs Implementation: Ensure all incidents are followed by the "Three Rs" process: Restore (well-being), Reflect (causes), and Revise (support plans). To report exceptions and non compliance to the operations directors and clinical director
  • Staff and Service User Support: Facilitate supportive debriefs for staff and people we support following any use of physical intervention.
  • Learning Integration: Ensure thematic summaries from learning debriefs are integrated into day-to-day practice and handed over across shifts.
  • 6. Statutory and Regulatory Framework The Clinical Quality Lead must ensure all care is compliant with the following UK legislation and guidance:
  • Human Rights Act 1998: Upholding Articles 2, 3, 5, and 8.
  • Mental Capacity Act 2005 (MCA): Ensuring the five statutory principles, including the presumption of capacity and best-interests decision-making.
  • Mental Health Act 1983 (amended 2007): Managing consent to treatment and detention frameworks.
  • Mental Health Units (Use of Force) Act 2018: Minimizing and monitoring the use of physical, mechanical, and chemical restraint.
  • Equality Act 2010: Ensuring no person is disadvantaged based on protected characteristics.
  • Social Services and Well-being (Wales) Act 2014 / Care Act 2014: Standardizing care and treatment planning.
  • NICE Guidelines [NG10, NG11, NG93]: Following best practice for managing violence, aggression, and challenging behaviour.

    Iris Care Group is seeking an experienced and values driven Intensive Intervention Lead Social Care to support the delivery of safe, evidence based care for people with complex mental health and learning disability needs. This senior role provides practice leadership, quality oversight, and crisis support, ensuring care aligns with national legislation, NICE guidance, and least restrictive principles., Extensive experience in delivering Positive Behaviour Support (PBS) for people with complex needs/challenging behaviour.
  • Proven track record in the application of Restraint Reduction Network (RRN) standards and principles.
  • Expert understanding of functional behavioural assessment and psychological formulation.
  • Deep working knowledge of the MCA (2005), DoLS, and the Human Rights Act (1998).
  • Ability to provide high-intensity crisis support and therapeutic engagement during behavioural disturbances., Experience working within a forensic mental health or high-complexity learning disability setting.
  • Experience conducting formal Restrictive Intervention Reviews.
  • Knowledge of NEWS2 physical health monitoring and SBAR communication tools.
