Full Job Description

We are looking for Integrated Support Workers who will travel across our local community, providing vital care and support in patients' own homes - helping people maintain their independence and live well where they feel most comfortable.

You will be working independently but as part of a team, traveling to patients/ clients homes to deliver care. You should have experience delivering care to patients/clients (nursing or social care setting) you will have a good standard of education which includes Maths and English GSCE (or the international equivalent)
