This is an exciting time to work in Cambridgeshire South as we accelerate our Neighbourhood Health Programme-an ambitious, system-wide effort to redesign how care and support is delivered across our Place.
The Integrated Neighbourhoods Project Manager will provide transformation support and project management to help develop Integrated Neighbourhoods and build relationships between organisations.
The Integrated Neighbourhood programme aims to co-develop a Neighbourhood Health Service that supports people to stay well in their own communities. Working in partnership with primary care, community services, local authorities, the voluntary sector, we are developing new models of support and care to deliver against the national Neighbourhood Health Framework.
We are looking for a Neighbourhood Project Manager to join our small innovative team for a up to a 12 month period for maternity cover, this can either be offered on a secondment or fixed term contract. This opportunity would be perfect for anyone keen to learn more about and support the development of Neighbourhoods across Cambridgeshire South area and more specifically in the South Cambridgeshire & Royston locality.
Your role will be to work collaboratively with partners and communities to align services and support, to guide the formation of neighbourhood teams, to recognise opportunities and identify how they can improve care and support, and then to deliver this change.
You will act as a relationship builder, a connector, a problem solver and a cheer leader, always promoting the neighbourhood ways of working at a time of significant pressures and challenges. You will provide transformation and project management expertise to make positive changes, build strong and sustainable foundations for Neighbourhood Teams and to tackle inequalities.

We are particularly interested in anyone with experience in the supporting the health and wellbeing of Children & young people and/or experience with Mental health services and support as this will be a key focus of this role. We are looking for someone who enjoys working with people and bringing the best out of them, a person with a confidence to work autonomously and opportunistically in a constantly evolving environment.
As this is a short-term position for up to 37.5 hours per week, secondments (please seek your managers permission before applying) and flexible working / reduced hours proposals are welcome., Provide transformation support and project management for the Integrated Neighbourhoods programme of work
Review population health data and support public engagement to determine how the local citizens want their care and support to improve
Be responsible for supporting the service redesign and delivery of projects within the Integrated Neighbourhood development team
Work closely to build relationships and bring partners and communities together to make positive change
Work closely in partnership with clinical leaders, the acute sector, community services, health commissioners, councils, voluntary and community organisations, and the public to support the development of the service design
Supporting the translation of a vision into a coherent operational model and project delivery
Promoting equality and reducing inequalities
Facilitating partnerships across all partners and supporting cross organisational and cross boundary working
You may line manage project support officers and will work closely and collaboratively with the other Integrated Neighbourhood Project and Programme Managers, At Cambridge University Hospitals, we want to do all we can to support good working days.
