Integrated Community Stroke Service Administrator

We are developing an Integrated Community Stroke Service (ICSS) within Great Yarmouth & Waveney, bringing together community stroke pathways into a more coordinated, responsive and patient-centred model of care. The role will involve inputting data sets on a regular basis to the sentinel stroke national audit programme (SSNAP).

This audit is a clinical and organisational audit to improve stroke care in the UK. The clinical audit is patient-level care tracking metrics for the integrated community stroke rehabilitation service and the organisational audit looks at structural and systemic resources to supply stroke services., An exciting opportunity has arisen within East Coast Community Healthcare (ECCH) to expand the participation and representation of ECCH services within the Sentinel Stroke National Audit Programme (SSNAP) with the development of the integrated community stroke service.

To provide professional, comprehensive and efficient management of collection and entering of SSNAP data.

To input data sets into the SSNAP database, ensure accurate recording of patient details relevant to the SSNAP domains.
