Full Job Description

We have been making some changes in the way we deliver our services across Primary Care, Community services and other organisations., The post holder will provide high-quality administrative support to the Integrated Neighbourhood Team (INT), ensuring smooth co-ordination of community-based, multidisciplinary services.

Working closely with clinical leads, social care teams, voluntary sector partners, and operational managers, the postholder will support the development of integrated working practices in community servicesand timely communication to improve patient outcomes and system flow.
