Full Job Description
The Specialist Community Public Health Nurse for Inclusion will support the development of pathways based on specific geographies and communities within Darlington in order to reduce health inequalities and improve health outcomes for vulnerable communities., The post will ensure service delivery reflects demographics, existing locality infrastructure and feedback influences service improvement. Alongside other practitioners within the Darlington 0-19 Service, the tSCPHN for Inclusion will be responsible for expanding outreach to those experiencing the greatest inequalities. The 0-19 Service, whilst retaining the delivery of the Healthy Child Programme, will have a specific focus on early intervention and prevention thus supporting a reduction in health inequalities and improved health outcomes.
