Full Job Description
An exciting opportunity has arisen within the Clinical Governance Team for an Incident Coordinator for a 6 month secondment. The post holder has a key trust-wide role in the work of the Governance Support Team, through the design, development and effective management and co-ordination of all administration services relating to Patient Safety Incidents (PSIs). Facilitating the management and operation of patient safety incident processes, including the analysis and presentation of data, reviewing and grading of PSIs, providing narrative and performance data for production of various high- level reports including the annual and quarterly reports to Board level Committees., Responsible for:
- Managing operation of service and PSI specific information systems
- Developing administrative working protocols and practices for trustwide implementation.
- Assist the Patient Safety Specialist and Patient Safety Team in developing, implementing and maintaining PSI specific policies, protocols and related systems.
- Coordinate the investigation of PSIs and monitor the progress of investigations to ensure responses are made within timescales.
- Proactively engage in policy development, providing and contributing information in line with the Patient Safety Incident Response Plan.
- Coordinate patient safety actions and learning response data and information for presentation at Trustwide meetings.
- Coordinate and minute meetings associated with PSIs and learning responses and provide in a timely manner.
- Ensure the Trust Risk Management system reflects the current status of all PSIs and associated learning responses and be able to present this information upon request.
- Proactively work with triage and grading processes to ensure timely responses for PSIs.
- Have a clear understanding of the potential implication of PSIs which may impact significantly on the Trust and be responsible for escalating incidents of concern following their disclosure.
- Develop and foster exceptional working relationships withservices on all matters relating to PSIs.
- The post holder will work to defined policies but is required to work on own initiative, utilising broad guidance as required
