Improvement Practitioner

• To support delivery of the Trust Improvement agenda through undertaking a range of improvement activities and supporting staff and teams to achieve sustainable improvements. To work collaboratively with teams across the Trust, utilising a range of improvement methodologies
• To deliver improvements through facilitation, engagement of stakeholders, analysis, planning and project management
• To contribute to organisational learning and the spread of improvement through training, coaching and the sharing of quality improvement tools, techniques and initiatives., Support teams across the Trust toidentify, plan, deliver and sustain improvement initiatives.
• Facilitate improvement activities using recognised quality improvement approaches,methodsand tools.
• Coach and support colleagues to develop their improvement knowledge, skills and confidence.
• Work collaboratively with staff, patients, service users and partners to understand challenges and develop solutions.
• Support the collection,analysisand presentation of data to inform improvement work and measure impact.
• Facilitate workshops, meetings and improvement events that engage and empower teams.
• Share learning, good practice and successful improvement approaches across the organisation.
• Contribute to the development of a culture of continuous improvement throughout the Trust.
• Build effective relationships with stakeholders at all levels to support the successful delivery of improvement programmes
