Hospital To Home Team Member

We are looking for a highly motivated individual who will assist in the delivery of Hospital to Home and Prevention activities; working collaboratively to drive change internally and further strengthen our Health partnerships as part of the PPR Strategy., As part of the role you must be committed to Safeguarding Children and Vulnerable Adults whilst carrying out your work. You will also be required to undergo an enhanced DBS check. We would also expect you to uphold the principles of the Core Code of Ethics., The Fluency Duty is outlined in the Code of Practice on the English language requirements for public sector workers. The Fluency Duty for this role is Required. You must be able to speak and provide advice in accurate spoken English to the public.
