Transfer of Care Hub in South and West Hertfordshire supports an integrated approach to managing the care of people who require support to leave hospital at the right time, with the right support., The role will involve working alongside Occupational Therapists, Community Care Officers, health colleagues and care providers to ensure people leave hospital as quickly and safely as possible. The role will involve supporting both the Discharge to Assess model to enable people to leave hospital as well as supporting people with complex needs through the completion of Care Act Assessments with an understanding of all relevant legislation and Continuing Healthcare. We promote personalisation, focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by enabling people to direct their own support. You will gain extensive experience in the field of adult social care, safeguarding practice and multi-disciplinary working., Our Integrated Discharge service in the West of Hertfordshire supports an integrated approach to managing the care of people who require support through the admission and discharge pathway, so the role will involve working alongside Occupational Therapists, Community Care Officers, health colleagues and care providers. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks. We promote personalisation focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by enabling people to direct their own support. You will gain extensive experience in the field of adult social care, safeguarding practice and multi-disciplinary working. About the role You'll ensure safe and timely discharges from hospital by:

  • Assessing the needs of patients and carers
  • Arranging appropriate care packages
  • Liaising with professionals and agencies
  • Supporting person-centred discharge planning

    Essential:
  • Social Work degree
  • Registration with Social Work England
  • Driving licence

English Fluency The ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post (for those whose language is a signed language the provision of a sign language interpreter who speaks English to the necessary standard of fluency will be required).
