East and North Herts Transfer of Care Team have an exciting opportunity for an experienced social worker/ AYSE completed social workers within the East and North Herts Transfer of care team. The team offers a range of opportunities to work within acute and community hospitals. Discharge to assess beds and are transfer of care assessment and community teams. Integrated Discharge Service Post Hospital Review Team Hemel Hempstead 37 hours per week | Permanent We are unable to accept AYSE / newly qualified Social Workers at this time Our Integrated Discharge service in the West of Hertfordshire supports an integrated approach to managing the care of people who require support through the admission and discharge pathway, so the role will involve working alongside Occupational Therapists, Community Care Officers, health colleagues and care providers. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks. We promote personalisation focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by enabling people to direct their own support. You will gain extensive experience in the field of adult social care, safeguarding practice and multi-disciplinary working., You'll ensure safe and timely discharges from hospital by:

  • Assessing the needs of patients and carers
  • Arranging appropriate care packages
  • Liaising with professionals and agencies
  • Supporting person-centred discharge planning
  • We offer a 7 day service and currently operate a 1-weekend-in-8 rota across the Hospital Team

  • Social Work degree
  • Registration with Social Work England
  • Driving licence

We are not able to offer sponsorship at this time for this role., You'll need to have the ability to drive and/or travel around Hertfordshire, either using your own mode of transport or a company car to travel to other locations. English Fluency The ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post (for those whose language is a signed language the provision of a sign language interpreter who speaks English to the necessary standard of fluency will be required).
