What You'll Do

  • Support adults with complex health and social care needs to promote independence and safe hospital discharge.
  • Screen and process referrals, gathering information from healthcare professionals and partner organisations.
  • Assist with reviewing care packages and developing outcome-focused support plans.
  • Attend meetings with health and social care professionals to support discharge planning.
  • Help complete assessments, maintain accurate records, and prepare reports and referrals.
  • Communicate with customers, carers and partner agencies by telephone, email and in person.
  • Support customers to access appropriate services, equipment and community resources.
  • Assist with risk assessments and help monitor and review customer support plans.
  • Maintain accurate case records and contribute to service performance monitoring.
  • Develop knowledge of adult social care, safeguarding and hospital discharge while working towards a Lead Adult Care Worker Level 3 Apprenticeship.


Training Outcome

Following successful completion of the apprenticeship, you can progress into roles such as Independent Living Assessor, Social Care Practitioner, Adult Social Care Officer, or other positions within Adult Social Care and Health services. There may also be opportunities to undertake higher-level qualifications and apprenticeships to further develop your career.
