Full Job Description
As a member of the Hospital at Home service, you will be expected to work as a senior nurse with a broad range of competencies who is able to adapt to a wide variety of clinical circumstances. To work as part of a multidisciplinary team to complete clinical assessments, treatments and management of patients within their own homes as part of the Hospital at Home and community- based admission avoidance pathways. The post holder will work autonomously within their sphere of competence, undertaking comprehensive assessments, formulating management plans, initiating treatment and coordinating multidisciplinary interventions to support patients with acute, sub-acute and exacerbations of long-term conditions.
