Full Job Description

Age UK Sheffield is looking for a compassionate, enthusiastic and creative individual to join our Hospital Discharge Team. This rewarding role supports older people during their hospital stay through meaningful activities, conversation and practical support. You'll help people stay connected, maintain confidence and independence, and feel more prepared for returning home. Working alongside hospital staff, patients and families, you'll play an important part in improving wellbeing, reducing isolation and helping overcome barriers that can delay discharge. What You'll Be Doing

  • Delivering engaging group and one-to-one activities on hospital wards.
  • Encouraging social interaction, confidence and independence.
  • Building positive relationships with patients and their families.
  • Helping people identify support available once they return home.
  • Working closely with hospital teams to support discharge planning.
  • Gathering feedback and measuring the impact of activities.
  • Supporting volunteers and students involved in the service.
  • Maintaining accurate records and monitoring information.

    We're looking for someone who:
  • Enjoys working directly with older people.
  • Has excellent communication and listening skills.
  • Is warm, patient and approachable.
  • Can work independently whilst being part of a wider team.
  • Is organised and able to maintain accurate records.
  • Can build positive relationships with health professionals, carers and families.
  • Has a flexible, creative and proactive approach to problem solving.
  • Experience of working with older people, activities, community support, health, social care or voluntary sector services would be beneficial, although we welcome applications from people who can demonstrate the right values and transferable skills.
