Hospice at Home Registered Nurse (Ashfield)

To work collaboratively with the Beaumond House team and the End-of-Life Care Together Integrated Hub to deliver a consistently high standard of hospice care in line with the organisations philosophy and values. The role focuses on planning, coordinating, delivering and evaluating person-centred, evidence-based palliative care for patients, families and carers., The Registered Nurse will support the effective delivery of the Beaumond House Hospice at Home service within their caseload, working closely with the multi-disciplinary team and partner agencies, contributing to staff and service development, and supporting admission avoidance and timely discharge to enable people to remain at home in accordance with their preferences and care plans .

Beaumond House Hospice supports patients and their loved ones across North Nottinghamshire, providing free, compassionate care at home and at our Hospice in Newark. We place each person at the centre of everything we do, offering physical, emotional, and spiritual support with dignity and respect. As a registered charity, we provide four in-patient beds, and Hospice at Home services for people with palliative and end-of-life needs. Our team offers personal care, emotional support, advice, and advance care planning in each persons preferred place of care. We are available 24/7 to offer information, a friendly ear, and signposting for patients, relatives, and carers.
